How would Paul Ryan's medicare plan affect persons on social security disability who get medicare?

I have a friend who is disabled with a life threatening and incurable illness and is often sick. He can not hold down a job and is by definition- disabled. But, he is in his 30's and so a far cry from 55. Chances are, he won't live to be 55 and even if he does he will probably still need medical assistance. Currently his medicare pays for everything. He doesn't have to deal with insurance companies or vouchers. Frankly, he wouldn't understand it even if he did and it would probably make him very confused and that could add to his illness. So, we are trying to understand how the miraculous miracle can't go wrong invention Paul Ryan came up with is going to deal with people like him.
